Fax/Phone Instructions

You can fill out the Application by two ways:

Print out the forms and then fill them out and sign, or
Fill out the forms first on the computer and then sign them. After you have signed the forms, please fax them to: (206)374-2159

Instructions:

1. Please fill out and sign the application form.

2. Please fill out the check:

Enter in your name as it appears on your checks.
Enter in your address as it appears on your checks.
Enter in the next check number that appears in your checkbook. The paper check will have to be voided so that there are no duplicates in your account. You can keep this voided check as a record of your transaction.
Enter in the date. The date must be the next payday after the date of the application.
Your check will be deposited on the loan due date or thereafter.
Enter in the amount you will owe. This amount equals the total payback amount. For example, if you apply for a $100 your total payback will be $115, on a $200 loan your total payback amount will be $230, etc. Please refer to the fee table.
Enter in your bank's name
Enter in your checking account information:
First enter in the bank's routing number, which is the first set of numbers on the bottom of your check between I: and I:
Then enter in your account number, which is the second set of numbers on the bottom of your check after the second I:
Sign your name

3. Proceed to fill out and sign the Note.

Enter the principal amount of the loan you are requesting, i.e. $100, $200, $300, etc.
Enter in the amount of fees, i.e. $15 for a $100 loan, $30 for a $200 loan, $45 for a $300 loan, etc.
Sign your name.
